Targeted dose naltrexone, also known as The Sinclair Method (TSM) uses standard dose naltrexone (50-100mg/d) in a specific protocol for the treatment of alcohol use disorder.

Thank you for scheduling your appointment with us to start your journey forward with The Sinclair Method (TSM).  To better assist you, below you will find the forms for your first appointment.

In person office visits

Visits in our Poway office are available Monday through Friday.  We offer the consultations at $150 per 30 minute block of time or $250 for one hour block of time.

If you would like to complete your intake forms in advance of your appointment, you may print them from the link below.  You may either bring them with you on the day of your appointment, or you may send them in advance of your appointment, either by fax to 858-312-6421 or email at  If emailed, please make sure that your forms are in pdf format. 

If you are planning to complete the intake forms in person on the same day as your appointment, please arrive a few minutes early.

Scheduling telemedicine appointments

Telemedicine consultations are available for patients in California.  Visits are available either by Skype or FaceTime.  On your paperwork or in your email please indicate which format you prefer and your contact information (Skype name or phone number for FaceTime).

At the time of scheduling your appointment, please select how much time you would like to reserve.  Follow-up visits are requested at the one month, three month, and six month marks from your initial consult.

Please print the following forms (total of 6 pages).  Only the first page of the arbitration forms needs to be printed.

Please complete the forms and email them to or fax them to 858-312-6421.  If you email the forms, they must be in pdf format only.  

There are some situations where naltrexone is not medically appropriate, including patients with liver failure or patients who are suicidal.  This is rare, but please keep in mind that your consultation is not a guarantee for a prescription for naltrexone, if the doctor determines that there is a medical reason why it is contraindicated.
